I have read and agree to FYNvet's Privacy Policy and Terms of Service. I understand how my information is collected, stored, and protected.

I authorize my assigned Financial Coordinator to access my contact information and my pet's care and billing records for the purpose of managing estimates, payments, and insurance claims on my behalf.

Allow my contact information and pet profile to be shared with other FYNvet-enrolled hospitals for continuity of care. If I visit a new FYNvet hospital, my coordinator can access my existing records. Payment information is never shared. I can revoke this at any time in my portal settings.

Authorize FYNvet to search connected veterinary software systems — including Cornerstone, ezyVet, AVImark, Impromed, and DaySmart Vet — to retrieve my pet's existing medical history on my behalf. This saves me time and ensures my new care team has complete information. I can revoke this at any time.

FYNvet is building insurance claim filing. When it is available, you will choose whether to let your coordinator file claims with your pet insurance provider on your behalf, and FYNvet will share only the information your insurer needs to process the claim.

Your practice retains ownership of all client and patient data. FYNvet processes this data solely to provide the services described in our Terms of Service. We do not sell, rent, or share your data with third parties for marketing purposes. We maintain encrypted storage, role-based access controls, and audit logs at all times.

I have read and agree to FYNvet's Privacy Policy and Terms of Service on behalf of my practice.

I acknowledge that my practice is responsible for compliance with applicable state veterinary privacy laws. FYNvet provides tools and workflows designed to support compliance, but my practice retains responsibility for adhering to the laws of the state(s) in which we operate.

If my practice is part of a multi-hospital group using FYNvet, I authorize shared access to client records across our affiliated hospitals for the purpose of coordinated care. Each client retains the right to opt out of cross-hospital data sharing.
